The Medicare coverage rules require the beneficiary and distant site practitioner to communicate via an interactive audio and video telecommunications system that permits real-time communication between the beneficiary and the distant site provider. This means the practitioner may not use audio-only, store-and-forward, or other message-based communications if the services are to be covered by Medicare. There is a minor exception allowing asynchronous store-and-forward technology in federal telehealth demonstration programs in Alaska and Hawaii.
